# Custom attendee forms

Create extra questions attendees must answer at checkout, set up per ticket type.

Custom forms let you collect extra information from attendees when they check out, such as T-shirt size, organization, or food preferences. These forms are set up **per ticket type**, right inside the ticket type form in the **Ticket Holder Form** section.

> Every ticket type already includes the **Name** and **Email** fields, which attendees must fill in. The custom questions you create are added below them.

## Creating a custom form

1. Open the **Tickets** tab, then create or edit a ticket type.
2. In the **Ticket Holder Form** section, click **+ Add Form**.
3. Fill in the **Field Label** (for example: Shirt Size).
4. Choose the **Field Type**: **Text**, **Choice (Dropdown)**, or **Checkbox**. For Choice/Checkbox, fill in the options.
5. Turn on **Required** if the question must be answered.
6. Save the ticket type. The question will appear on the checkout page for attendees who buy that ticket type.

## Applies per ticket type

Questions only appear when an attendee buys the ticket type where the form was created, which suits specific needs like a VIP ticket. You can add up to 15 custom questions per ticket type.

> **You can change it even when the event is active**

> You can still add, remove, relabel, or change whether a question is required even after the event is **Active**. Removing a question doesn't delete answers attendees have already given, so your data is safe. The only thing that's locked is **changing the field type** (for example, Text to Choice) once an attendee has answered it, because that would change the meaning of the data already collected.

## Field types & required status

Choose the field type that fits the kind of answer you need: **Text** for free-form answers, **Choice (Dropdown)** for one answer from several options, or **Checkbox**. Each question can be marked **Required** or optional.

> Required fields make sure important data is always filled in, but too many required questions can put attendees off finishing checkout. Ask only what you really need.

Attendees' answers are saved with the order data so you can use them to prepare for the event. All the answers are also included when you [export the attendee list](/en/organizer/attendees-and-check-in/exporting-the-attendee-list) to Excel, with one column per question.
